An Overview of the Delivery of your Care
A Field Supervisor or Manager will visit you in your home and undertake an assessment of your needs. They will carry out a care assessment with you to identify what support you may require at your home, or to meet your needs. A copy of this assessment will be sent to you.
This will be reassessed every year unless prompted sooner. Our office staff will liaise with the Field Supervisor to create a care plan for our staff to follow during the care visits.
The Field Supervisor will conduct a Risk Assessment to ensure that all work can be undertaken safely in your home. If required, a ‘Moving and Handling Risk Assessment’ will also be undertaken to identify any equipment and/or handling techniques needed for care in your home. A copy of these assessments can be posted out to your home address. Our office staff will liaise with the Field Supervisor to prepare a risk assessment for all the care staff to view within your home.
Once both yourself and Hearts at Home Care have agreed the type of care required, your documents will be uploaded to a software called Carebeans, this system allows care staff to electronically view your assessment of needs and care plan. Your care pack will be for our staff to read thoroughly and have records of yourselves for our staff to update such as: – Daily Log Sheets, Medication Records (if needed), your care plan that was agreed on the first visit and any risk assessments. You are more than welcome to monitor this with our office staff. Hearts at Home Care staff work alongside a phone app ‘Carebeans’ to provide a ‘greener’, paper light care company, so this will mean that most of your details will be held securely by digital format.
Your care pack will be re-assessed every year or earlier if needed to provide you with the best possible care. We work in Ringwood, Verwood, Fordingbridge, Alderholt, Bransgore, Christchurch, Wimborne and surrounding areas.
